OPTIMIZATION OF OSTEOCHONDRAL ALLOGRAFT PREPARATION FOR TRANSPLANTATION USING FINITE ELEMENT MODELLING

Many factors have been reported to affect the functional survival of OCA transplants, including chondrocyte viability at time of transplantation, rate and extent of allograft bone integration, transplantation techniques, and postoperative rehabilitation protocols and adherence. The objective of this study was to determine the optimal subchondral bone drilling technique by evaluating the effects of hole diameter on the material properties of OCAs while also considering total surface area for potential biologic benefits for cell and vascular ingrowth.Using allograft tissues that would be otherwise discarded in combination with deidentified diagnostic imaging (MRI and CT), a model of a large shell osteochondral allograft was recreated using LS-PrePost and FEBio based on clinically relevant elastic material properties for cortical bone, trabecular bone, cartilage, and hole ingrowth tissue. The 0.8 mesh size model consisted of 4 mm trabecular bone, 4 mm cortical bone, and 3 mm cartilage sections that summed to a cross-sectional area of 1600 mm2 (40 mm x 40 mm). Holes were modeled to be 4mm deep in relation to clinical practice where holes are drilled from the deep margin of subchondral trabecular bone to the cortical subchondral bone plate. To test the biomechanic variations between drill hole sizes, models with hole sizes pertinent to standard-of-care commercially available orthopaedic drill sizes of 1.1mm, 2.4 mm, or 4.0 mm holes were loaded across the top surface over a one second duration and evaluated for effective stress, effective strain, 1st principal strain, and 3rd principal strain in compressive conditions.Results measured effective stress and strain and 1st and 3rd principal strain increased with hole depth.The results of the present FEA modeling study indicate that the larger 4.0 mm diameter holes were associated with greater stresses and strains within OCA shell graft, which may render the allograft at higher risk for mechanical failure. Based on these initial results, the smaller diameter 2.4 mm and 1.1 mm holes will be further investigated to determine optimal number, configuration, and depth of subchondral drilling for OCA preparation for transplantation

Bone graft tuberculosis outbreak in USA: Is it a concern in India?

Globally, 25% of the population is infected with tuberculosis, which poses a leading cause of death worldwide. The transmission of tuberculosis (TB) during organ transplant is reported in the literature whereas only one report has been published on the transmission of TB, during bone allograft transplantation. In the US, in May 2021, an outbreak of TB occurred in patients undergoing spine surgery with bone allograft. This bone graft was retrieved from 80 years deceased donor with latent TB, which was not diagnosed earlier. The recipients were started with a long course of anti-tuberculous drugs. This review narrates the pathway of TB spread among transplant recipients and the strategies to be followed while performing organ or tissue transplantation.

Transient activation of notch signaling enhances endogenous stromal cell expansion and subsequent bone defect repair.

BackgroundFollowing traumatic bone loss or removal of bone tumors, the failure of bone allograft transplantation for large bone defect repair remains a significant problem in orthopedics. Therefore, new strategies that can efficiently enhance allograft healing and long-term incorporation are critically needed.MethodIn this study, we first injected Notch-activating Jagged1 peptide to mice and then isolated bone marrow tissues and cells for proliferation and differentiation assays. Femur bone allograft surgery was also performed in Jagged1 pre-treated mice, and bone defect healing process were monitored by histology, Micro-CT and biomechanical testing.ResultOur results showed that Jagged1 therapeutic injection is sufficient to maximally activate Notch and promote bone marrow stromal cell proliferation in vivo, while no effects on bone structure were observed. More importantly, Jagged1 pre-treatment significantly promoted bone callus formation and increased bone mechanical strength during allograft healing in a femur bone defect mouse model.ConclusionThis study reveals that Notch in vivo activation can be induced by injection of Jagged1 peptide for expansion of local native stromal cells that will significantly enhance bone callus formation.The Translational potential of this ArticleThe clinical uses of this therapeutic strategy would be immediately applicable for chronic long bone defect repair. More importantly, this devised strategy for expansion of endogenous BMSCs can also be applied to enhance other tissue and organ repair.

Periosteum Mimetic Coating on Structural Bone Allografts via Electrospray Deposition Enhances Repair and Reconstruction of Segmental Defects.

Structural bone allograft transplantation remains one of the common strategies for repair and reconstruction of large bone defects. Due to the loss of periosteum that covers the outer surface of the cortical bone, the healing and incorporation of allografts is extremely slow and limited. To enhance the biological performance of allografts, herein, we report a novel and simple approach for engineering a periosteum mimetic coating on the surface of structural bone allografts via polymer-mediated electrospray deposition. This approach enables the coating on allografts with precisely controlled composition and thickness. In addition, the periosteum mimetic coating can be tailored to achieve desired drug release profiles by making use of an appropriate biodegradable polymer or polymer blend. The efficacy study in a murine segmental femoral bone defect model demonstrates that the allograft coating composed of poly(lactic-co-glycolic acid) and bone morphogenetic protein-2 mimicking peptide significantly improves allograft healing as evidenced by decreased fibrotic tissue formation, increased periosteal bone formation, and enhanced osseointegration. Taken together, this study provides a platform technology for engineering a periosteum mimetic coating which can greatly promote bone allograft healing. This technology could eventually result in an off-the-shelf and multifunctional structural bone allograft for highly effective repair and reconstruction of large segmental bone defects. The technology can also be used to ameliorate the performance of other medical implants by modifying their surfaces.

Optimizing indications of impacting bone allograft transplantation in osteonecrosis of the femoral head.

The aim of this study was to report the medium-term outcomes of impaction bone allograft and fibular grafting for osteonecrosis of the femoral head (ONFH) and to define the optimal indications. A total of 67 patients (77 hips) with ONFH were enrolled in a single centre retrospective review. Success of the procedure was assessed using the Harris Hip Score (HHS) and rate of revision to total hip arthroplasty (THA). Risk factors were studied, including age, aetiology, duration of hip pain, as well as two classification systems (Association Research Circulation Osseous (ARCO) and Japanese Investigation Committee (JIC) systems). After a mean follow-up period of 8.61 years (SD 1.45), 81.3% (52/64) of enrolled cases had a good or excellent HHS at latest follow-up (declining to 76.0% (38/50) for those with more than eight years of follow-up). Overall survival was 92.1% at eight years' follow-up (95% CI 83.2% to 96.4%). A total of 12 hips (19.0%) failed (reoperation or HHS < 70 points) at final follow-up. Rate of success was adversely affected by increasing age, duration of pain, and more severe disease as measured using the ARCO and JIC classifications, but not by aetiology of the ONFH. We report favourable medium-term results of this procedure. Best outcomes can be expected in patients matching the following indications: younger than 40 years; less 12-month hip pain before surgery; femoral head collapse being less than 2 mm; and integrated lateral wall of femoral head. Progress in clinical treatment of large segmental bone defect

Clinical treatment of large segmental bone defect has always been a major challenge for the medical community, which is due to the complex and diverse pathogenic factors of large segmental bone defect.In recent years, the clinical treatment of large segmental bone defect has made great progress, and the main treatment options include vascularized or non-vascularized autologous bone grafts, allograft bone transplantation, masquelet technology, llizarov technology and bone tissue engineering.Therefore, understanding the advantages and disadvantages of various treatment options is very important for the clinical treatment of large bone defects in long bones, laying the foundation for clinical treatment. [Progress in repair and reconstruction of large segmental bone tumor defect in distal tibia].

To review the methods of repair and reconstruction of the large segmental bone tumor defect in distal tibia. The related literature of repair and reconstruction of the large segmental bone tumor defect in disatal tibia were reviewed and analyzed from the aspects of the reserved ankle joint and the non-reserved ankle joint. The large segmental bone tumor defect in distak tibia is not rare in clinical. In addition to conventional allograft bone transplantation, vascularized autologous fibular transplantation, vascularized fibular allograft, inactivated tumor regeneration, distraction osteogenesis, and bone transport techniques; membrane-induced osteogenesis, artificial tumor stem prosthesis, three-dimensional printed metal trabecular prosthesis, ankle arthrodesis, artificial tumor ankle joint placement surgery are now gradually applied to the repair and reconstruction of large segmental bone defects in the distal tibia. Moreover, due to its long survival time, the function of reconstruction of the bone tumor defect in the distal tibia has also received increasing attention. Although the ideal methods of repair and reconstruction of the large segmental bone tumor defect in the distal tibia has not yet been developed, great progress has been achieved. Recently, with the appearance of three-dimensional printing and various preoperative simulation techniques, personalized and precise therapy could become ture, but therapies for the large segmental bone tumor defect in the distal tibia still need to be further explored.

[Discussion on hot topics of skeletal sternal reconstruction].

Sternum is an important constituent of the thoracic cage, and a pivotal part of the anterior chest wall. When a considerable portion of the sternum needs to be resected, in order to protect the vital organs beneath, restore the anatomical structure and physiological function, reconstruction is unavoidable. Sternal reconstruction consists of two related parts: skeletal and soft tissue reconstruction. Skeletal reconstruction uses materials such as autograft or allograft of bones, metallic plates, sandwich patch, non-rigid patches, bio-synthetic patches and three-dimensional printing prosthesis. Each has its advantages and disadvantages. For example, autogenous bone transplantation is limited by the source of donor bones and surgical trauma; allograft bone transplantation demands cryopreservation and is susceptible to infection; titanium plate cannot be placed overlapped; "sandwich" patch is susceptible to local fluid accumulation and infection; non-rigid patches cannot provide enough mechanical support; the design and manufacture of three-dimensional printing prosthesis is complicated, time consuming and expensive, and cannot be adjusted due to change of extent of resection during operation. The modularized prosthesis system is a promising new technique developed on the basis of titanium plate. It is divided into standard components of different sizes that can be selected according to operational requirement and easily assembled. Until now, no perfect method or material has been found to imitate the autogenous sternum. Soft tissue reconstruction is a prerequisite to the success of large scale skeletal reconstruction of the sternum.

Automatic allograft bone selection through band registration and its application to distal femur.

Clinical reports suggest that large bone defects could be effectively restored by allograft bone transplantation, where allograft bone selection acts an important role. Besides, there is a huge demand for developing the automatic allograft bone selection methods, as the automatic methods could greatly improve the management efficiency of the large bone banks. Although several automatic methods have been presented to select the most suitable allograft bone from the massive allograft bone bank, these methods still suffer from inaccuracy. In this paper, we propose an effective allograft bone selection method without using the contralateral bones. Firstly, the allograft bone is globally aligned to the recipient bone by surface registration. Then, the global alignment is further refined through band registration. The band, defined as the recipient points within the lifted and lowered cutting planes, could involve more local structure of the defected segment. Therefore, our method could achieve robust alignment and high registration accuracy of the allograft and recipient. Moreover, the existing contour method and surface method could be unified into one framework under our method by adjusting the lift and lower distances of the cutting planes. Finally, our method has been validated on the database of distal femurs. The experimental results indicate that our method outperforms the surface method and contour method.

Bone Regeneration: Microparticulate and Biomimetic Strategies

Structural allograft bone transplantation has evolved over many years and despite improvements in fixation and advances in graft safety, there are significant limitations and potential complications. This has prompted investigations in our laboratory to reduce bone to its smallest common denominator and to combine the inductive potential of small particle allograft bone with somatic stem cells and inorganic matrices as a regenerative engineering strategy for skeletal reconstruction. Preliminary animal studies identified an optimal particle size that since has been corroborated in human trials for closed segment bone defects. Further studies have confirmed the invigorating effects of stem cells cultured with bone particles that too have been observed and confirmed in human transplantation. The addition of structural inorganic materials to particulate bone and cells presents an alternative to large structural allografts. Newer technologies that alter the surface properties of inorganic and organic matrices offer unique biologic and physical pathways to bone induction that may be transformative in the field of bone regeneration and provide superior clinical alternatives to segmental bone loss. A retrospective study on annual evaluation of radiation processing for frozen bone allografts complying to quality system requirements.

Bone allografts have been used widely to fill up essential void in orthopaedic surgeries. The benefit of using allografts to replace and reconstruct musculoskeletal injuries, fractures or disease has obtained overwhelming acceptance from orthopaedic surgeons worldwide. However, bacterial infection and disease transmission through bone allograft transplantation have always been a significant issue. Sterilization by radiation is an effective method to eliminate unwanted microorganisms thus assist in preventing life threatening allograft associated infections. Femoral heads procured from living donors and long bones (femur and tibia) procured from cadaveric donors were sterilized at 25kGy in compliance with international standard ISO 11137. According to quality requirements, all records of bone banking were evaluated annually. This retrospective study was carried out on annual evaluation of radiation records from 1998 until 2012. The minimum doses absorbed by the bones were ranging from 25.3 to 38.2kGy while the absorbed maximum doses were from 25.4 to 42.3kGy. All the bones supplied by our UMMC Bone Bank were sterile at the required minimum dose of 25kGy. Our analysis on dose variation showed that the dose uniformity ratios in 37 irradiated boxes of 31 radiation batches were in the range of 1.003-1.251, which indicated the doses were well distributed.

Bone Allografts In Dentistry: A Review

Transplantation of bone allografts is an accepted procedure in dentistry as it is in many surgical specialties. Despite wide acceptance and ready access to a number of bone allografts, there is often insufficient knowledge of the origin of these allografts and the processing methods. This brief review paper summarizes contemporary knowledge of the biologic properties of bone transplants used in dentistry and discusses their safety. It is intended to aid dental practitioners in selecting suitable bone allograft materials for their patients. It does not deal with bone autografts nor does it compare autografts and allografts. Long-term clinical results with allografts processed by different methods are also outside of the scope of this review.

Desmoplastic (collagenous) fibroma of the femur: A case report and review of the literature

Desmoplastic fibroma is a rare, benign soft-tissue tumor composed of spindled and stellate-shaped cells that are embedded in a dense collagenous stroma. Clinically, desmoplastic fibroma presents as a firm, mobile, slow-growing mass that is located in the subcutaneous tissue or near the deep aspect of the skeletal muscles. The present study describes the case of a 66-year-old female who presented with an inactive, firm, slightly tender mass in the lower medial segment of the right femur. An open biopsy was performed and the result of the pathological examination indicated a desmoplastic fibroma. The patient underwent a radical resection of the tumor and the accompanying bone, which was then reimplanted using devitalized tumor bone, self-ilium graft and homologous allograft bone transplantation, with an internal fixation by locking the compression plate. This was followed by a reconstruction of the anterior and posterior cruciate ligaments and the lateral and medial collateral ligaments. There was no evidence of local recurrence at five years post-surgery.

Surgical treatment of fibrous dysplasia in the proximal femur

The aim of this study was to summarize oncological and functional results and to investigate surgical treatment methods and efficacies by conducting a retrospective study of patients with fibrous dysplasia (FD) in the proximal femur. A total of 15 patients with FD in the proximal femur were selected. Among them, 12 cases were monostotic and 3 cases were polyostotic. In addition, 2 cases were accompanied by shepherd’s crook deformity. All cases received internal fixation following focus curettage and impaction grafting. Among them, valgus osteotomy was conducted for 2 cases of shepherd’s crook deformity. All patients were followed up for 12–32 months. For 2 patients with shepherd’s crook deformity in the proximal femur, the collodiaphysial angle was recovered after an orthopedic procedure following osteotomy. In addition, no patient presented with postoperative recurrent lesions. At 3 months following surgery, local bone resorption was visible in the bone grafting area. Between 8 and 12 months after surgery, the bones in the bone grafting area had healed, pain had disappeared and gaits were nearly normal. An effective internal fixation following thorough focus curettage and allograft bone transplantation is an effective method of treating FD in the proximal femur. For patients with shepherd’s crook deformity, it is also necessary to perform valgus osteotomy to recover hip joint function.

Triple Red Blood Cell Alloantibody Formation After Bone-Allograft Transplantation

In this case report, we provide evidence for the possibility of red blood cell alloimmunization after bone-allograft transplantation. Here, we present a 13-year-old boy who received a bone allograft due to impending hip-luxation. Five months later he was shown to have developed three different alloantibodies: anti-D, anti-C and anti-E, which were induced by the bone allograft. Red blood cell alloimmunization is a possible adverse event when a patient is exposed to allogenic red blood cells. These antibodies may cause transfusion reactions when incompatible blood is administered. More importantly, these antibodies may cause severe, or even fatal, hemolytic disease of the fetus or newborn, stretching the importance of preventing antibody formation, especially in young women. This case demonstrates the importance of selecting rhesus phenotype compatible bone allografts.

Gamma Irradiated Bone Allografts Processed from Femoral Heads

Disease transmission and bacterial in fection in bone allograft transplantation is of significant concern. Screening of donor for d isease, bacterial testing and aseptic processing, substantially reduce risk, but do not co mpletely eliminate the possibility of allograft associated infections. Sterilization by gamma rad iation is a defin itive method for eliminating microorganis ms and can prevent life-threatening allograft associated infections. In the present study, microbiological evaluation of bone allografts processed fro m femo ral heads excised during surgery was carried out. 126 femo ral heads obtained from living donors were processed, freeze-dried and sterilized by gamma irradiation at 25 kGy . The bioburden and type of microbial contamination associated with bone allografts was determined based on morphological characteristics and biochemical tests. Resistance of bacterial contaminants to gamma radiat ion was evaluated by exposing the bacterial cell suspension to different doses of 1 to 6 kGy and analysing for the surviving bacteria. Average bioburden of p rocessed bone allografts for different batches was found to be in the range of 1.84 x 10 2 to 3.88 x 10 3 CFU/g. 60.2% o f the isolates were found to be Gra m-positive organis ms. The D10 value of bacterial isolates ranged fro m 0.56 to 1.68 kGy. Verificat ion doses for different batches of processing were 5.87 to 9.46 kGy . All bone grafts exposed to the verification dose were tested culture negative. The results validate 25kGy dose for sterilizat ion of bone allografts processed fro m femo ral heads.

Allograft bone transplantation for neuropathic arthropathy of the knee associated with type 1 diabetes mellitus

Diabetes mellitus causes neuromusculoskeletal disorders characterized by abnormalities of nervous tissue, joint and bone. Early diagnosis and prevention of disease progression is difficult in cases of neuropathic arthropathy of the knee in diabetes. We report the case of a patient with type 1 diabetes mellitus who developed an insufficiency fracture of the medial part of the proximal tibia, which was viewed as an early finding of neuropathic arthropathy of the knee. In surgical treatment of the fracture, allograft bone transplantation and internal fixation were performed after curettage of the pathologically fragile lesion. Postoperatively, radiological findings have showed complete allograft bone incorporation and no evidence of degenerative changes. Recognition of an insufficiency fracture of the knee as an early indicator of neuropathic arthropathy and prompt treatment of the fracture using allograft bone transplantation could result in joint preservation.

Cartilage restoration in haemophilia: advanced therapies

Current treatment of joint cartilage lesions is based either on conventional techniques (bone marrow stimulation, osteochondral autograft or allograft transplantation) or on newly developed techniques (chondrocyte implantation and those based on cell therapy that use bioreactors, growth factors, mesenchymal stem cells [MSCs] and genetically modified cells). The aim of this article is to review the therapeutic strategies above mentioned and to determine whether the chondral damage seen in haemophilia could benefit from any of them. The different conventional techniques have shown similar results whereas autologous chondrocyte implantation, which is in common use at the present time, has not been shown to produce any conclusive results or to lead to the formation of hyaline cartilage. MSCs hold promise for the repair of joint cartilage given their differentiation capacity and the therapeutic effect. The use of bioreactors and growth factors, which stimulate cartilage formation, may optimize such strategies in the context of reimplantation of chondrocytes, differentiated MSCs and cartilage progenitor cells. The aim of cell therapy is restoration of function through the repair of damaged tissue or the stimulation of growth factor synthesis. Implantation of autologous chondrocytes or MSCs was up to now able to address only highly localized chondral lesions. Adequate control of the differentiation process as well as the use of growth factors and appropriate bioreactors could transform cell-based therapies into a more efficient and longer term treatment even for patients with haemophilia. Nevertheless, raising false expectations in these patients should be avoided. There are a number of approaches to cartilage restoration in haemophilic arthropathy, which are currently being explored for other joint related degenerative disorders. If it can be proven to be effective for the disorders in which clinical trials are ongoing and costs could be limited, it might be an useful palliative approach to haemophilic arthropathy. However, we still have a long way to go for use in haemophilic arthropathy.
